How to overcome communication problems

I'm a 56 yr old woman, working full time. My coworkers, who are intelligent, social people, as well as my boss, tell me I ramble too much off topic and interrupt others inappropriately. I am on Concerta 72 mg, had tried Adderall which made me too jittery. I feel that, in order for me to be able to build relationships with others, I need to somehow overcome this behavior. I've been told, "Think before you speak" -- easier said than done. My therapist says " That's the way your brain is wired." That is not a consolation--am I forever unlikeable and friendless because of this? I feel perhaps a therapy group would help me, but there is none within a 100 mile radius of where I live. Please, I am desperate and very frustrated. What can I do to get help and to help myself? Carol
