Math teacher won't follow 504 plan!

OK I need some advice in regard to my daughters 504 plan. It is for her ADD, and visual/auditory processing disorder. The main issue is that specifically for MATH class she is to have less problems on a page to allow more room to to do the work (not less homework problems just more space on the page) well her math teacher has told my daughter at the start of school that she would do it later because she already had class sets made up, I called and spoke with her and told her nicely that that was not adequate. So my daughter still comes home with the homework not set up as her 504 plan states, and I get an email from the teacher telling me its too hard and she doesn't know what to do and her idea is to email me the homework so I can print it out...well if she can't adjust the size of the same file, how does she expect me to do it? More importantly its HER JOB!!! I nicely gave her some suggestions and advised that she speak with last years math teacher and/or the science or history teachers as they don't seem to be having an issue. Of course no reply...and my daughter is STILL coming home with homework that is not following her 504 plan. I am frustrated and don't want to be the constant complainer BUT this is affecting her learning and her ability to do her homework in a timely manner because she has no space to do her work. My child is frustrated and has said she feels stupid because she can't get the work done. I did leave a voicemail today for the assistant principal to call me as she is in charge of the 504 plans this year. I need suggestions!
