Medication that somewhat helps...

I have the same two problems - and mine were caused by menopause (at the age of 44).
I take Lamictal for the bipolar and that has helped quite a bit. (You need to see a good psychiatrist for that medication management. DO NOT get this med from your regular doctor.)
The ADD is the most frustrating for me. For that, I'm taking Seroquel - the smallest dosage made (1/2 of a 25mg pill). The really seems to help - although I had to get used to the initial "hangover" effect.
My problem isn't the focus: it's deciding on a focus. I'm a writer and I have a horrible time deciding which avenue of my writing business I want to pursue (e.g., books, articles, user guides, press releases, etc.). I have found that I have to commit to one and surround myself with reminders that this is what I'm focused on right now. When other things start to creep in, I push them to the side and make a concerted effort to ignore them. I also see a license prof'l counselor (LPC) who helps me to weed out all the other "fluff" stuff that my brain doesn't need.
Have you tried looking for some support groups in your area? That might help as well.
