Here's some really simple advice. Don't try to think of things to put on the 504, instead, do what you've just done and list what the problems are that need to be addressed. Bring something in writing, that shows why you think these things are issues for him (teacher's comments like "needs to stay focused in class" and missing homework assignments really help to outline that) what you have done outside of school and at home to address those issues, and let the school suggest ways to deal with them. Then start negotiating. As soon as YOU suggest something, things are likely to go askew. Bring them the problems, and what you've done to do your part in addressing them, and lay it at the school's feet to come up with solutions. That way, the school will really get behind IMPLEMENTING the 504. Getting the 504 is only part of this, getting the school to actually implement it is likely to be the bigger obstacle in the long run, and it's easier to be smart about it in the beginning and set this(and your kid) up for success.
