Concerta +?
My 11 yr old takes 54 mg, and when it stopped working his doctor added 2 mg Tenex (filled as generic Guanfacine). It has worked very well, for over a year. Apparently this blood pressure medicine has been used since the early 90's for this and has been used with children. He used to add 10mg Ritalin and the end of the school day, since Concerta's effects wore off in 8 hrs, but as he got older we didn't neede this anymore. Also, during allergy season, my son can't take Zyrtec. It completely makes the Concerta ineffective for him, but a children's liquid allergy medicine (Equate) works and doesn't cause sleepiness so he takes it befofre school. Track everything that's going on and communicate with the doctor. Good luck.
