Here's what I used....
.....this year when I requested accomodations for my son this year. I did not send it to the principal or the teacher or counselor but to the Director of Exceptional Children's program (not sure what you school district uses). You would not have BELIEVED the awesome response I got and we have already had 2 meetings with the principal, teacher, assistant principal, counselor, behavioral therapist and school psychologist and my son is now getting the help he DESPERATELY needed from school - including being moved to another teacher and classroom (current teacher is a bit older, set in her ways and had absolutely no patience for my son from day one. Unfortunately he could not be moved until the first grading period is completed 11/1/2010 and his current teacher has graded him so harshly and sent home ALL his unfinished classwork AND homework home to complete (takes us HOURS) that he is now becoming angry, aggressive and disheartened. Even the principal was surprised by the notes and "attitude" coming home about a 7 year old. Luckily his new teacher has a son ADHD, is the professionally trained "classroom intervention" expert in the school and very structured. I have not been this hopeful since my son started school.
