ADHD or something more? Helpless, lost and frustrated! HELP!!!

My daughter is 9, almost 10. She was diagnosed with ADHD in kindergarten. I knew something wasn't "right" when she was the early age of 4. I noticed when I would try to talk to her about something important she was NEVER able to maintain eye contact with me. That was my first sign. Of course she was hyper...she was 4! She was fortunate enough to have the same teacher for pre-k and k. She has always worked above grade level and done well in school. In kindergarten her teacher noticed her inability to pay attention and focus had increased from prior year. It started affecting her grades and this is when we met with pediatrician and started her on Focalin-xr 5mg. She has always been very outgoing and seems to have friends at school, but not a lot outside of school. She has been on the Focalin since moving up to 15mg. This summer I noticed she seemed depressed so to say. She would have these emotional melt downs. She has always seemed to have the melt downs but they are getting worse!!! So the pedi suggested trying a different class of med..we started Adderal...BIG MISTAKE! She became even nastier than before. Our whole house suffered. We also have a 12yo boy and 2.5 yr old girl. She would even say she didn't like the way Adderal made her feel and asked to be back on her old meds. So when school started we switched back to Focalin. Things just seem to be getting worse. Whether on meds or off if something doesn't go her way she has the "melt down" This consist of ugly looks, negative body language, begging for what she wants and the absolute worst part is she cries uncontrollably. To the point where she slobbers and it's as if she reverts into a small child. There is no talking to her when this happens. If you try to reason with her in any way it's useless. So many times when she is upset and even when she's not I ask her to look me in my eyes when I'm talking to her and it's very obvious she can not do even this simple thing. We have tried so many different ways to deal with this. She definitely gets plenty of attn. I feel like we could of just come home from Disney and she would not be happy b/c there isn't a friend to play with right at that time. it seems as if she has no filter when it comes to the things she says to people or the things she talks about. No matter how many times you remind her. Sometimes it's embarrassing and I'm so afraid people will judge her on that. Some people I know already do. On a positive note she is a dancer and does very well with it. She enjoys that. She is so sweet at times and loving. The "melt downs" are becoming more frequent. It's tearing our family apart and I feel so helpless. I don't know if this is "normal" adhd behavior or if she may have something else going on. I've read about the ODD and she really doesn't fit into that. It's killing me to not be able to help my daughter. Anyone have any similar problems? I feel so alone in this battle. I know i'm not but her "episodes" don't seem so ADHD related. We have not seen a psychiatrist yet...that is next. Thank you for taking the time to listen to me vent :)

I am so sorry you are having such a tough time right now. If your daughter wasn't having these outbursts before medication, it very well could be a side effect of a medication that just isn't right for her. Not all children can tolerate all medications. It is good to question if there is something else going on with your daughter as well. As Dr. Silver stated in the article I referenced above, sometimes stimulants bring out symptoms of a mood disorder or other diagnosis that weren't evident before. You mention things like a lack of recognizing social cues and not being able to look someone in the eye that can be a symptoms of something else entirely.

Of course, you should always seek the advice of the prescribing doctor when you feel like you have experiencing trouble with prescriptions. Maybe it's a good time to also reevaluate all of your daughter's current symptoms to find some resolution.

Hi ScMom ---I have 8 year old son who has long been diagnosed as ADHD, and has been on ritalin for 2 years. It did help, but the mood issue has become more pronounced over time also. He was started on prozac, which helped his anxiety. Now we are exploring (at urging of psychiatrist and psychologist) whether he might also have a mood disorder. There are lots of books on the subject--apparently distinguishing ADHD and mood disorders is very tough, and the two occur in the same kids frequently . We are trying to wrap our minds around this for our gorgeous son, but we are mystified at the changes in mood--from raging to sobbing and back, and so so so irritable. As far as I can tell, the irritability is a key to the mood disorder diagnosis. If you just have sadness, I would still see a psychiatrist, but first keep a journal (or chart) of moods or temper tantrums, how long they last, etc The doctor will need to know this. I wish you good luck, and I totally get how frustrating the process is !
