Middle school help?

This is my first post... My daughter (11) has inattentive type ADD. Elementary school was not helpful in setting up a section 504 plan with her. Their reasoning was that she didn't qualify because she understands concepts, makes good grades (A's and B's with a very occasional C), etc.. Her main issue is staying focused and getting the work done & turned in on time. Not to mention coming home with all the materials she needs. I did not press the issue (I probably should have, but do not have a very aggresive personality) because her grades have been good. Next year she starts middle school and I am SO concerned about her being able to keep up. How do I get her the help she needs when schools don't seem to want to devote the time it needs to help her. I actually had a teacher say "I have 22 students. I don't have time to focus extra attention on one". No one has seemed eager to help us. My daughter is EXTREMELY shy and won't ask for teachers help in class. I would really like to have a set of textbooks at home. Does a 504 plan have to be in place for that to happen? Do I write a note to each of the teachers? Have the Dr. write a note? Thank you for your input!
