dosing for 72 mg concerta

i'm 44 and was diagnosed officially with add earlier this year. i tried strattera and had bad side effects. now i've been on concerta for a few months. i'd been on a dose of 54 mg but noticed it didn't seem to last beyond a few hours. my doctor upped the dosage to 72 mg, or basically two 36 mg pills.

my question is about the initial effects of taking the 54 mg vs. the two 36 mg pills. i had some of the 54 left and take them on the weekend when i don't feel i need a full days. there seems to be a noticeable difference in the initial delivery. i feel the effect of the 54 mg much more in that i am much more focused right from the start. when i take the two 36 mg, i don't notice the same level of focus at any point.

has anyone else noticed this effect? any ideas why this would be the case? is it something about the delivery mechanism in the different doses? is there a dosing strategy for taking the 54 mg initially and boosting it sometime later in the day when the effects diminish?

sorry if this has been covered before. i tried searching but didn't know quite how to phrase it to bring up any previous threads on this topic.

54 - 72ml concerta

When I switched from 54 to 72 ml of concerta each morning i noticed a big difference in the length of time it was taking to work.i.e. motivation and confused thought process. There's something not the same between the two doses. Although there supposed to be time realesed over 12-18hrs, I beleave the smaller doses are releasing quicker into our system as opposed to the 72 ml dose. I've asked my specialist and he is currently looking into the problem but no solution yet. If he replies i will be sure to let u know the outcome. (I'm a 34 year old male.)

54 Concerta + 20mg Ritalin

Before switching to Concerta, my dosage for Ritalin was about 30mg every 3 hours during work hours. The inconsistency/variability/ups&downs of this dosage led me to change to 54mg Concerta supplemented by 10 to 20 mg of Ritalin every three hours. I find this works well for me as the Concerta provides a base level effect that is sufficient for easier workloads (ie. meetings, training people, or working at home). For work requiring more focus (writing, budgets, communications), I supplement with 20 mg of Ritalin every 3 hours (ie. 9am, 12pm, 3pm). I like this as I can adjust (reduce) my Ritalin dosage based on my needs for that day while the Concerta provides enough effect to keep me generally productive. When I have to work in the evenings (a little bit too often), I'll take 20 mg Ritalin which is usually sufficient, eventhough the effect of the Concerta has mostly worn off.

I did try two 36mg Concerta once (my son takes one/day - yes, it runs in the family), but didn't like the effect.
