Angry Versus Sleepy on Meds

My son has anxiety disorder, A.D.H.D., and a slight autism diagnosis. For half of last year he took 5 mg. Ritalin twice a day. He became more angry and rude, but got his work done faster. This year he started Middle School. I thought a long-acting stimulant would be good so he wouldn't have to take time from class to go to the nurse to get his medication.. We tried Adderall XR, but he was too sleepy in school, and would almost fall asleep. Now we are trying Concerta 18 mg., he is awake, but he is more rude and angry. He also takes 5 mg. Prozac. More Prozac makes him rude and angry. How can I have a focused, polite and awake kid in school?

As an ADD/HD adult that was diagnosed as a child, I've pretty much tried them all. I took Ritalin before I took Adderall, and made the switch because of the "Permanent Road Rage" that I felt. Any little issue (like traffic) which would normally make me annoyed, all of a sudden made me furious. The long-acting Adderal did nothing for me, and the short-acting I had to take 4x a day (I need for school & study when I get home). The short-acting did work for me, but I had to take every 4 hrs, and that wouldn't help your son much. I'm on Vyvanse now, hating the price, not sure if I'm on the right dose (crappy HMO) or not, but overall, it is smooth as opposed to the "highs" and "lows" when the Adderall or Ritalin begins to work/crashes. There are no highs/crashes on it for me. Also, has to be taken in the morning, and that's it. My friend's son is on Vyvanse, and seems to be doing well on it. If he's sleepy on meds, it just might be the wrong dosage. Getting the right meds/dosage is the issue I always have, too-and remember that they all work differently for each person. Hope this helps.

Adderall

That's interesting that long acting adderall didn't work for you, but short acting did. I will keep that in mind. Ritalin and Concerta both made him angry. Vyvanse made him stay up all night. So right now he is on Adderall long acting, 5 mg. We tried going up to 10mg and he stayed up all night. So Adderall seems to be the best option. I will ask his psychiatrist about short acting adderall.
