Welcome to the ADDitude forums and thanks for your post. A work history full of fits and starts is common among ADDers. Knowing this may not help you find a stable next job, but at least you should know you're not alone.
First, I would recommend reading some of ADDitude's best articles about job hunting with adult ADD. I particularly like Find a Job and Love It and Job Hunt Tips for ADHD Adults.
Also, have you considered doing freelance graphic design work? Every day, you can find hundreds of ads on craigslist from people who need short, quick projects completed by a graphic designer who can live across the country or across the world. They don't care so much about your work history as they do about the quality of your work. This might allow you to beef up your porfolio and get back into the swing of things for a few months before seeking full-time employment.
