Strange behavior around different surroundings and people

My grandson is 6 years old, diagnosed with ADHD beginning of school year, is now on Adderall XR 10mg and has been on that dosage for a couple of months. Was started out on 5 mgs. Also on Clondine(?) for sleeping problem, .05mg 2 hours before bedtime. He does pretty good in school but the problem I have been noticing is that when he goes to the doctor, the minute she walks into the room he begins to act up. He starts getting loud, wanting to kick and hit at me, not wanting to sit still for an exam, will not let us talk by interrupting, being loud, etc. He does not do this with the nurse. He has been going to this doctor for over a year. And at Thanksgiving, his uncle came for dinner and he was sort of obnoxious, wanting to climb all over his uncle, wanting his attention, being disruptive, loud, etc. When he sees his uncle at work, which is a fast food restaurant, he does not act like this. I am stumped as to what is going on and how to handle these two situations. Christmas is coming up and I do not want a repeat of Thanksgiving, and his next doctor's appointment is the end of December. Thanks for any help.
