Dr Appt Tomorrow - ? To Ask

We're taking my 10 yr old grandson tomorrow to the dr. We have the couselor report - school input - about answering a question in class then when the teacher turns back to him he's doing something completely different. Can't go to sleep, can't make friends, always know the answer, tunes you out, does impulsive things, BUT he's such a loving boy the other times. What do we ask the dr to make sure we get the right help? His father ignores him - but not his older bro & sis - just gone thru a divorce - but he's had lots of positive enforcement with Grandpa & his uncle. I'm afraid they'll just focus on the divorce and blame it on that. His father shows alot of the same symptoms.
