ADHD and anal drilling

My name is Sarah. I am a mother of 3. My oldest, Connor, is almost 6. He was diagnosed with severe ADHD combined type with ODD last Nov. He has been taking Focalin XR 10mg in the morning with a Focalin 5mg booster at 4pm. He was doing great up until now. He has been having severe sleeping issues, i.e. not falling asleep until midnight or later. This along with old bad behavior have been resurfacing. The biggest issue is this awful new thing his has been doing. First, let me explain something. When I say he can't sleep, it's not that he can't, it's that he doesn't try. He sneaks out of bed and hides toys in his bed and plays. Now, lately, he has been getting out of bed(or staying in bed) and sticking his finger up his rectum and smearing poop on the wall or the floor. He isn't pooping his pants, although accidents in his pants have become regular during the day, he is digging the poop out of his rectum. This happens at least twice a week, if not more. He shares a room with his younger brother who is 3. Last night he woke his little brother up at 1am and turned on the light to get him to play with him. He then went downstairs and proceeded to get all the fruit snacks out of the pantry and take them upstairs and eat them. He then went downstairs again and got dirt out a plant and dumped it in the bathroom sink. He played in the bathroom sink long enough to clog the drain and dump my husbands contact lenses down the drain. He then went back upstairs and did the poop thing on the carpet. He was soooo quiet doing this that we didn't hear him until 3am. After being punished, I took my younger son out of the room and put him in bed with my Husband. I slept upstairs with him. At 5am, he tried to get out of bed again.

I took him to the doctor a few days ago for a med check. Doc said he will probably need his meds adjusted, but he wants his to see a psychiatrist first. The only doc he can see in his insurance can't see him until August. I can't wait this long. Have any of you had any similar experiences or heard of any?

poop smearing

I have heard of a lot of this kind of thing in autism specturm kids. I hope psych can help. A developmental neuropsych seems to be best for differential diagnosis.
