The daily report card can focus on one problem area or on several for a child with attention deficit disorder (ADD ADHD). Each behavioral target should be self-explanatory or explained to the child in advance, such as: "Accepts feedback in an appropriate manner."
Here are a few examples:
Classwork
Starts work with four or fewer reminders
Completes assignments with 70% accuracy
Contributes to class discussion with three or more original and relevant comments
Conduct
Interrupts class fewer than three times
Remains seated with four or fewer reminders
Teases classmates fewer than three times
Responsibility
Gives DRC to teacher
Has materials necessary for class
Brings completed homework to class
