The daily report card can focus on one problem area or on several for a child with attention deficit disorder (ADD ADHD). Each behavioral target should be self-explanatory or explained to the child in advance, such as: "Accepts feedback in an appropriate manner."
Here are a few examples:
Classwork
- Starts work with four or fewer reminders
- Completes assignments with 70% accuracy
- Contributes to class discussion with three or more original and relevant comments
Conduct
- Interrupts class fewer than three times
- Remains seated with four or fewer reminders
- Teases classmates fewer than three times
Responsibility
- Gives DRC to teacher
- Has materials necessary for class
- Brings completed homework to class
